Support Coordination Level 3
Our Level 3 Specialist Support Coordination service helps people with more complex support needs. We help when there are bigger challenges, many services involved, or support from different people needs to work together. We work with you to understand your NDIS plan, solve problems and make sure your supports are working well for you.
Step 1
Approved NDIS Plan
You need an approved NDIS plan that includes Specialist Support Coordination funding.
Step 2
Service Agreement
You will have the opportunity to meet with an Elektos representative, ask any questions and then sign a service agreement so you understand the support we provide and how we will work together.
Step 3
Understand Complex Needs and Supports
We help you understand your plan, your goals and the supports available to you. We also help with more complex situations where extra support may be needed.
Step 4
Coordinate Services and Solve Challenges
We work with you and your providers to organise supports, manage challenges and make sure services work well together. We also help you feel more confident in making decisions about your supports.
